2023 - 5k Run and Walk, Waiver & Release Form

Please carefully read all terms below before electronically signing. This document affects your legal rights. A written copy of this agreement can be provided to you upon request by contacting the Human Resources Representative at your respective company.

As a participant in the 2023 Atonne Family Wellness 5K Run/Walk (“Event”), acknowledge that my participation in the Event involves a risk of injury, including bodily injury, and assume the risk for same. On my own behalf and on behalf of my heirs and legal representatives and to the fullest extent permitted by law, I hereby release and discharge Atonne Group, Inc. and their respective directors, officers, employees, affiliates, volunteers, agents and representatives, of and from any and all liability for injury, death, or damages and/or any other claims, demands, losses or damages, incurred by me in connection with any aspect of the 5k run and walk.

If I am an employee of Atonne Group, Inc., I acknowledge that my participation in the 5K Fun Run and Walk is completely voluntary and does not constitute part of my work-related duties. I understand that my decision to participate, or not participate, in this activity will not affect my job status.

Photo/Video Consent and Release. I hereby authorize the Atonne Group and its affiliates to take photographs, recordings, and/or videos (whether electronic, digital, or otherwise) in connection with the Event. I hereby consent to the use, reproduction, and publication of such images. I, individually and/or in my capacity as parent/legal guardian of Participant (as applicable), hereby waive any right to inspect or approve the actual use by the Atonne Group, of any such image of Participant. Such images of Participant shall be the sole property of Sponsor, and I, individually and/or in my capacity as parent/legal guardian of Participant (as applicable).
